Great Books Volume 1: Friendship and Fellowship

I hereby grant permission for my child/participant to fully participate in all activities at the Great Books Summer Institute from August 7-8, 2026.  I understand that all activities or events will take place under the guidance and supervision of faculty, staff, and students of Franciscan Missionaries of Our Lady University.  Authorization and permission is hereby given to furnish all necessary transportation, food, and lodging for my child/participant.

I hereby certify that to the best of my knowledge, my child/participant is in good health, and I have listed in detail in the appropriate section above any known allergies and physical accommodations my child/participant may have.

In the event of an emergency, I hereby give permission to transport my child/participant to the appropriate level of care facility and hereby authorize any necessary medical treatment including, but not limited to, injection, anesthesia, or emergency surgery. I, notwithstanding any question of liability involved in seeking urgent or emergency medical care, fully and completely assume responsibility for all medical and, if necessary, emergency transportation costs. I understand that if I am unreachable after all possible attempts that you may contact the listed emergency contact. I give permission for the release of my child/participants medical information to medical professionals if emergency medical attention is warranted. Should it be necessary for my child/participant to return home due to medical reasons, disciplinary action or otherwise, I assume all responsibility and transportation costs.

I understand that upon arrival at the retreat site, all daily medications, vitamins, and supplements will be released into the possession of the retreat nurse for the duration of the week, except for rescue medications including, but not limited to, asthma inhalers and epi-pens. I give permission for non-prescription, over-the-counter medication to be given to my child/participant by the retreat nurse or adult staff at his/her discretion for minor illnesses including, but not limited to, headaches, nausea, diarrhea, minor cuts and scrapes, etc.

I hereby agree to indemnify and hold the Great Books program, Franciscan Missionaries of Our Lady University, the FMOL Health System and its respective members, directors, employees and agents (collectively, the "Indemnitees") harmless from and against any and all claims, demands, actions, lawsuits and liabilities, including attorney's fees and expenses sustained by the Indemnities as the result of the negligent, willful or intentional acts of my child/participant (regardless of age).
